(CNN) -- Heath Ledger died as the result of an overdose of prescription medication, the New York City medical examiner's office said Tuesday.

"Mr. Heath Ledger died as the result of acute intoxication by the combined effects of oxycodone, hydrocodone, diazepam, temazepam, alprazolam, and doxylamine," the office said in a short statement.

"We have concluded that the manner of death is accident, resulting from the abuse of prescription medications."

Ledger died January 22 at an apartment in Lower Manhattan. The Oscar-nominated Australian actor, best known for his role as a stoic, closeted cowboy in the 2005 film "Brokeback Mountain," was 28.

An autopsy done on the actor January 23 was inconclusive.

A housekeeper, identified as Teresa Solomon, arrived at the apartment about 12:30 p.m. on the day Ledger died, a police source with knowledge of the investigation said.

She saw Ledger lying on a bed face down with a sheet pulled up around his shoulders and heard him snoring, the source said.

Masseuse Diana Wolozin arrived at the apartment about 2:45 p.m. to give Ledger a massage, according to the police source. About 15 minutes later, when he had not come out of the bedroom and the door remained closed, she went in, saw him lying in bed and set up a massage table.

She shook Ledger, but he did not respond, so she used his cell phone to call actress Mary-Kate Olsen, a friend of Ledger's, in California, the source said.

Wolozin told Olsen that Ledger was unconscious, according to the police source.

Olsen reportedly told her that she would call private security people in New York.

At 3:26 p.m., Wolozin called 911 and told authorities Ledger was not breathing. While on the phone with dispatchers, Wolozin tried to perform cardiopulmonary resuscitation on Ledger, but he was unresponsive.

Emergency personnel arrived seven minutes later, according to the police source, at about the same time as a private security person summoned by Olsen.

The medical technicians performed CPR on Ledger and used a cardiac defibrillator, but their efforts were in vain and he was pronounced dead at 3:36 p.m. By then, two other private security people summoned by Olsen had arrived as well as police.

Ledger's family called his death "very tragic, untimely and accidental." Relatives returned home Tuesday to Perth, Australia, where a private funeral ceremony reportedly will be held, according to The Associated Press. The timing of the funeral is unclear, the AP said.

His former fiancée, actress Michelle Williams, has asked the public to respect the need for her, the couple's 2-year-old daughter, Matilda, and others "to grieve privately."

"My heart is broken," Williams said in a statement issued last week via her publicist. "I am the mother of the most tender-hearted, high-spirited, beautiful little girl who is the spitting image of her father. All that I can cling to is his presence inside her that reveals itself every day. His family and I watch Matilda as she whispers to trees, hugs animals, and takes steps two at a time, and we know that he is with us still. She will be brought up in the best memories of him."

Condolences poured in from Ledger's friends and co-stars.

"He was a wonderful guy, he was a wonderful actor, he had a wonderful future ahead of him, and I liked him," said actor Eric Roberts, who worked with Ledger in "The Dark Knight," the latest installment in the "Batman" series set to open in July.

Colleagues on the Terry Gilliam's film "The Imaginarium of Doctor Parnassus," which Ledger had been shooting in England, said the actor apparently had been suffering from a respiratory ailment in the days before he died.

Christopher Plummer told Entertainment Weekly that Ledger had a "terrible, lingering bug in London, and he couldn't sleep at all. We all -- I thought he'd probably got walking pneumonia."

Ledger's first American film was the teen comedy "10 Things I Hate About You" in 1999. He passed up several scripts before taking a role in the Revolutionary War drama "The Patriot" in 2000 and "A Knight's Tale" in 2001. He also played a supporting role in "Monster's Ball."

But Ledger was perhaps best known for his portrayal of Ennis Del Mar in "Brokeback Mountain," Ang Lee's film about two cowboys who had a secret romantic relationship. The role earned Ledger a best actor Oscar nomination.

Anybody can get addicted to the kind of meds he was taking.  My mom was addicted to three of the six at one time or another.  Plus, they said it wasn't like he took a whole bunch of pills at once.  It was a toxicity that built up over time.  And he did have pneumonia when he died of respiratory failure.  This is just a terrible tragedy that happened to a very nice family.

A Knight's Tale was just suddenly there on TBS tonight.  I got sucked in.  I've wanted to watch it again for a while.  Fortunately, I've been able to enjoy most of it and feel how fun and joyful it is.  But during the Golden Years dance, it hit me.  He's really gone.

Wow, have you guys seen this?

A guy named Vincent Fantauzzo, a friend of Heath's from Australia, painted it not long before he died.  Heath not only sat for it, they created the concept together and Heath repeatedly played all three versions while Vincent worked on it.  Vincent says Heath thought of the figures as "consciences".  And said that people could decide for themselves what the voices were saying.

PILL TRAIL: Heath Ledger overdosed on OxyContin and other drugs at his SoHo apartment. Feds are looking into how he got those drugs.

Pint-size actress Mary-Kate Olsen has refused to be interviewed by federal investigators probing the accidental drug death of her close friend Heath Ledger unless she receives immunity from prosecution, The Post has learned.

The actress' lawyer has repeatedly rebuffed attempts by the feds to question Olsen, who was the first person called after her masseuse discovered Ledger's body in his SoHo apartment in January.

Frustrated federal officials could obtain a grand-jury subpoena to compel the funky "Full House" actress to tell them whatever she knows about the "Dark Knight" star's behavior, his possible drug use and the events of that fateful morning, according to sources.

Probers have interviewed everyone connected to Ledger and his death, including his doctors, the masseuse, bodyguards, housekeepers, business associates and even the mother of his 2-year-old daughter, Matilda, his "Brokeback Mountain" co-star Michelle Williams.

"Ms. Williams was extremely nice and cooperative," a source said.

Another added, "Everyone has been very eager to help, saying what a great guy Heath Ledger was, everyone except Mary-Kate, who has refused to speak."

That source explained that Olsen would be the final witness they need to conclude their investigation into where he got his drugs and medicines.

Ledger - whose performance as the Joker in "The Dark Knight" has ignited Oscar buzz - died of a potent cocktail of prescription drugs and OxyContin, the latter of which was likely obtained illegally.

His death was ruled an accidental drug overdose by the medical examiner.

And NYPD investigators closed their probe without interviewing Olsen because, they said, they got all the cooperation they needed from others. That decision, however, surprised some observers, considering Olsen's role as a potential witness.

Instead of calling emergency responders after getting the call from her masseuse, Olsen telephoned her bodyguards in the Big Apple, telling them to race to Ledger's Broome Street home, where they arrived just as paramedics got to the scene.

The US Drug Enforcement Administration, which opened an investigation to determine the sources of Ledger's drugs, was not as sanguine as the NYPD about talking to the 4-foot-11, 90-pound Olsen.

The feds contacted her lawyer, Michael Miller, several times recently seeking her cooperation, but Miller has told the feds Olsen won't be interviewed unless she gets immunity, exercising her constitutional rights.

According to sources, all of the drugs in Ledger's body and discovered nearby in prescription bottles were legally obtained from two physicians - with the exception of OxyContin, a powerful painkiller.

Investigators "are trying to ID the source of the OxyContin," a source said. "Did it come from a dealer, from a friend? If he had a bottle from a friend, was it taken by someone else before police responded? That is what is trying to be determined."

The DEA probers were also aggressive with the NYPD, the sources said, and were forced to threaten to obtain a grand-jury subpoena before getting the department's Ledger files.

An NYPD spokesman said the department never balked at handing over the files and did so after receiving a routine federal administrative subpoena.

Asked whether he asked for immunity for his client, Olsen's lawyer said, "We are not going to comment on whether there is a criminal investigation," and declined to speak further.

The sources said Ledger's employees reached out to Olsen because they panicked at finding his body and did not know what to do.

As for the bodyguards, they claimed they responded only to offer help.

Right after he died, someone said they were having a fling.  There's really been nothing official.  The only two people who would know for sure are Heath and Mary-Kate and MK's not talking.
